a one week incubation period suggests viral response. Can you make a differential of virus vs. mycoplasma based on this incubation period?

Consider that North Korea has next to no people movement in and out of the country. It is extremely isolated. If NK were to launch an attack against the world, say in Singapore, NK would be one of the last countries to be affected by the pathogen and the epidemic might have burned out by then.

No.

Can you make a differential of virus vs. mycoplasma based on this incubation period? Not based on incubation in this case, but chest x-ray and positive culture will give you your differential

Not really.
Mycoplasma's "incubation period" is really difficult to identify. Mycoplasma chlamydia could be thought to have a very long incubation period because some victims do not show ANY signs of the infection. This means that a completely asymptomatic person can spread the disease to a sexual partner.
