More likely they didn’t bother using sufficient rebar (due to corruption at the engineering or construction level, and then corruption at the inspection level). That’s more often the case since once the concrete is poured, it’s next to impossible to inspect what’s inside of it (since concrete does very well in blocking x-rays).
I had a case of a parking lot where the plans were clear as day to have 16” spacing for the rebar, but they laid-in 24” spacing. It was pretty obvious when I saw it, and the pile of remaining rebar that was over half of what we purchased. Ended up having a crew remove all of the rebar to prevent the contractor from trying a 2AM pour (since concrete companies won’t pour a slab if they don’t see any rebar).
