I read through the full article, and didn’t see any mention of the specific period this investigation covered. For all we know, these failures could have happened under Obama’s administration. You can’t tell, because the writer failed to answer as many of the required facts: who, what, where, when, why, and how...that they could.

8% isn’t something to brag about. That is a bit more than the casualty rate of troops landing on Omaha beach on day 1.
If they grabbed 100 random letters, and 8% were a problem. That is an absolute breakdown and a crisis.
