What if the doll is marketed as a youthful-looking midget? As disgusting as this is, I don’t think the law is enforceable.

As disgusting as it is, it will be tossed by the courts. Back about 10 years ago the Federal Government passed a law slapping the same penalties on "depictions" of child pornography (cartoons, computer generated images and the like) as real child pornography. The Supreme Court tossed the law, saying such depictions were protected free speech because unlike real child pornography no actual illegal act took place in its production.

Wouldn’t that be akin to saying cartoon images of a murder should be treated as an actual murder?

And now an end-around has been done around that ruling because somehow a doll is different than a drawing in regards to being an inanimate object that has no rights.

With this kind of bizarro-world ‘logic’ I might be charged someday with murder for killing computer-generated characters (NPC’s) in Fallout 4.
