Airline safety is measured by AirlineRatings.com, which uses a variety of metrics to award airlines as many as seven stars for safety and quality. The below airlines are the only ones in the world with only one star.
1. Air Koryo This may not be particularly surprising, but North Korea's national airline is rock bottom in the world rankings. Unlike most entries on this list, it is fatality-free and has an IOSA Certification, which is an internationally-recognized standard for the airline's operation and control systems. So, why the low star rating?
For one, Air Koryo received an EU ban in 2006 due to safety concerns about their equipment. While parts of their fleet has begun complying with international standards and thus been allowed to operate internationally, the majority of their planes remain banned. Also, Air Koryo's planes were made in Russia, which automatically costs you a star in AirlineRatings's score.

That's hilarious that you automatically lose a star if you fly a Russian built aircraft.

You could lose more than a star!
the mechanical devices the Russians ever got right were the T-34/76 tank and the AK-47.
that saying about broken clocks comes to mind . . .
The Nepal Airlines have an additional problem in that they have to fly in and out of Lukla Airport. One way in, one way out, very high, and an uphill runway.

I would have guessed Aeroflot
Yeti Airlines?
What about it's American subsidy, Sasquatch Air?
I have lived dangerously!
Flew on a Czech airline (Russian plane) from Mumbai to Prague in 1960. I don’t recall the model, but I do remember a few of that particular model have crashed.
Flew on a Air India plane in 1968 from London to Mumbai, with 3 stops on the way. Every time the plane would descend to land, I had water dripping on my head. Good thing I was carrying an umbrella because the plane was full.
Flew from Singapore to Manila in 2013 on Philippines Air
where I had very bad case of food poisoning. Barely made my next flight to San Francisco at Manila airport because I was terribly weak.
Best food I ever had on a flight was on Air France from Paris to Stockholm.
Aeroflot has undergone a major makeover. They aren’t Singapore or Dubai status by any means but reliable. Brand new planes and European service with hints of Soviet attitude.
“I would have guessed Aeroflot”
Actually Aeroflot flies mostly western built airplanes today. They have a bunch of Boeing 737’s & 777’s along with Airbus 320’s and 330’s. I’m an airline pilot and I see them around the globe in various airports, they’ve changed into a decent airline since the fall of the Soviet Union.
Also, their flight attendants are the hottest in the world. Watching a gaggle of them walk through a terminal is like watching a bunch of Victoria’s Secret models before VS started with the fat acceptance thing.

Amelia Earhart
I can only imagine what Amelia would have said about that low T, no ability loser. Good thing there are psych departments available to handle those who’d fail at McDs. (No insult intended to McDs, maximum insult intended for the Ford bimbo.)
Since Communism fell, 199 of their 248 aircraft are either Airbus or Boeing.

Forget Blue Wing airlines out of Suriname and fly Azul out of Brazil with their model pilots.
I flew on Air India from New York to Heathrow and back in 1985, I think it was. Horrible. I was sure we were going to die.
A couple years ago I flew on Laos Airline and the planes were so old they still had ashtrays in the armrests.
You sure get around!
