Clinics which offer low-cost medical choices with payment based on ability to pay would provide competition. If people didn't have insurance that covered their costs, they'd think real hard about what's worth it and what isn't. With a larger market, low cost alternatives would be found.
It isn't rocket science. It's capitalism.
