I have a compromise - they can't try to enlist until after they have had all their treatments/surgeries and have been healthy (physically and mentally) for two years. They have to remain 100% deployable or face discharge like anyone else who becomes undeployable.
If they enlist and then 'suddenly decide" they want to mess with their parts, they can either put it off and live as their birth sex or be discharged so they can pursue their mutilations.
I'd wager it would have the same effect as not allowing them in at all.....
