Why are the strange characters showing up?

That has been going on here for years (strange keyboard characters replacing certain type(s) of apostrophes. But very rarely have I seen the black diamonds with question marks inside. Almost always, when it happens, the replacement characters are a string of 3 or 4 odd keyboard characters.

Below are examples of both types of apostrophe replacements. And it seems to only involve one type of apostrophe, the slanted one, as opposed to the perfectly vertical one ( ' ).
