Pry it and my Bible from my cold dead hands. I don’t answer those dumb questions on doctors forms, no more than I do questions I find offensive.
i come across the “is there a gun in the home” occasionally. My answers have ranged from “no” (it’s on me belt or in my car when I’m not home)to “piss me off and find out.”
Such questions are none of their business. And, like an employer asking you a question that’s illegal to ask, courts have ruled time and again that lying as an answer is just fine if the question itself isn’t legally allowed.
