I agree that few of these are true lone wolves.

I believe that a considerable number (Sandy Hook, in particular) had someone else pulling the strings. It is pretty easy to groom someone who is mentally unstable to commit a rash act. It may be hard to control the details, but getting them to go on a shooting spree or something similar would not be difficult. You just need to play into their need to feel important. This is similar to how cults acquire members. (like the democrat party, for example).
