So if the States did not set up an exchange the people in said state will go to the made up ones by Sebillius and obama which is Medicare or Medicaide?

Seems like it is a made up program to me and I am wondering if these people will really get insurance or if it will just be government stuff.

No no no.
I am not saying that at all, and no that is not going to happen.
But, the Federal Government is using the offices of Medicare and Medicaid, and the SAME employees of said agencies, to train and enforce the rules on those involved with Obamacare.
