I am inclined to not suspect al-Qaeda as much as some other, far more capable, player.
For instance, Iran has really gone to lengths to preposition al-Quds (Quds Force), which is active in dozens of countries and even has eight major directorates, or regional commands.
They have had some success in “setting up shop” in other countries, but have been singularly incompetent in carrying out terrorist acts or even simple assassinations. Likely because their organization has been penetrated by several intelligence agencies.
So, while their theoretical threat is considerable, practically speaking, they are bozos.
