What does shiny fingers mean nutritionally?

Nursing home patients sometimes have the same "waxy" look on their hands/fingers. Not sure what the cause is. Lack of movement/use? Atrophy?

I think the 4th year is the length of time he'd been there. He was captured in '07 and the pics were supposedly taken in '11.
