If a semi-auto pistol has a full magazine inserted and one in the chamber, when it is fired even once it will no longer have a full magazine. Every time.

Yes, but many people then pull the magazine out and put another bullet in. That’s the “15+1” you see in descriptions as to how much ammo the gun can hold - the 1 is the one in the pipe.
