I wonder if night vision goggles were involved. Don’t they tend to reduce depth of field? No details yet, was it a head on or side to side collision?

So sad this happens when we have such great technology and training...

That likely is the issue. As for NVGs, there is reduced depth perception and field of view, the tradeoff of course is being able to see at all.
