There’s always the possibility that he os a “Claudius.” whom everyone thought was a dummy. Of course, Claudius was a scholar despite his speech impediments, which probably allowed him to survive. The Julian family was the familily from hell.

If the top photo is indeed his(which is not, but let us entertain that possibility,)
it looks like somebody squeezed his head into a square box, and pressed hard. His face is flattened, top of his head, and the side of his head too. His jaw looks more square as well.
