State Dept adding intrusive, semi-impossible questinnaire for US passport applications

Olga sez, "The U.S. Dept of State is proposing a new Biographical Questionnaire for passport applicants: proposed new Form DS-5513 asks for all addresses since birth; lifetime employment history, personal details of siblings; mother's addresses prior to your birth; any "religious ceremony" around time of birth, circumstances of birth including names (as well as addresses/phone numbers) of persons present, & more. Failure to answer can mean denial of passport, & govt reserves right to use this info for 'routine uses.'"

Update: Commenters note that this form is specifically intended in lieu of a birth certificate with a passport application; but as the FA suggests, the circumstances in which people unable to provide a birth certificate will be given this form (rather than the traditional bureaucratic investigation) are not spelled out; further, the form itself remains a Kafkaesque impossibility for most people to complete.

It seems likely that only some, not all, applicants will be required to fill out the new questionnaire, but no criteria have been made public for determining who will be subjected to these additional new written interrogatories. So if the passport examiner wants to deny your application, all they will have to do is give you the impossible new form to complete.

It's not clear from the supporting statement, statement of legal authorities, or regulatory assessment submitted by the State Department to the Office of Management and Budget (OMB) why declining to discuss one's siblings or to provide the phone number of your first supervisor when you were a teenager working at McDonalds would be a legitimate basis for denial of a passport to a U.S. citizen.

The proposed new Form DS-5513 asks for all addresses since birth; lifetime employment history including employers’ and supervisors names, addresses, and telephone numbers; personal details of all siblings; mother’s address one year prior to your birth; any “religious ceremony” around the time of birth; and a variety of other information. According to the proposed form, “failure to provide the information requested may result in … the denial of your U.S. passport application.”

Working for the DOD and NASA I’ve had a bunch of different Security Clearances.

None of them required me to list any addresses before I was 18.

A passport is bloody handy for domestic travel in the airports these days. When you show your passport you typically blow right through the TSA inspection. Not being able to obtain a passport confers upon you a kind of second class citizenship.

Does anyone for even a moment believe Obama would provide the answers to Question 9 of the form without his typical non-response (accepted by media and the majority of politicians in office and running for office?) 9. Did your mother receive pre-natal or post-natal medical care?

or how about Questions 10, 11 and 12?

10. What type of document, if any, did your mother use to enter into the United States before your birth?

12. Was there any religious or institutional recording of your birth or event occuring around the time of birth? (Example: baptism, circumcision, confirmation or other religious ceremony. Please provide details including the name, location of the institution, and date.)

If you are going to get a passport, do it now before this becomes real.

Interesting. I didn’t know that since I haven’t been in our country for about 7 years.

LOL I’d be able to fill this out.My DM only had two addresses before my birth and I remember where all I’ve lived.I’ve also been a housewife for the past 25+ years.

They are going to have to hire half of the U.S. to checkup on everyone’s background who has applied for passports.

It is true. I know the propaganda says that everyone is treated the same at airports but the fact is that if you have a passport you save a bunch of time answering stupid questions. A passport is also awesome when you need ID at a bank or in similar situations and what’s bets about it is that none of the numbers on a passport link to your financial identity the same as your driver’s license number does. So if someone copies your passport numbers they really don’t have anything. So I use it all the time instead of my driver’s license when I need ID.
