I hope you’re joking; no one can be that dumb. He hit his head, the skin bled, and either the blow caused an intra-cranial bleed or the bleed was what caused the fall.

I am not that informed about patients who are on dialysis but is it possible he was on a blood thinner of some sort? If he fell and injured his head while taking a medication that slowed clotting time, it seems it could cause a small injury to do a lot of damage very quickly.
