In all fairness, plenty of times. McCain lost four planes AFAIK, one of them being attributed to pilot error, i.e. 'his fault'. Second plane was lost due to engine failure. Third plane was lost on the deck of the USS Forrestal in '67 when another plane actually fired a missile in error, striking the plane NEXT to McCain's, which triggered the fire which claimed 134 lives. The fourth plane was the A-4 McCain was in when he was shot down over North Vietnam.

McCain was praised by the Navy for his "aggressiveness and skillful airmanship" and awarded him the Navy Commendation Medal in '67.

For my money, McCain should have stayed in the Navy.
