Uhmmm... Maybe the fact that an inmate managed to overpower two guards and escape with their weapons?

What are the odds that the guards in question were either female, overweight, grossly out of shape, or some combination of the above

It should be a job requirement that a guard is in better condition than the inmate. Perhaps we should also take away the inmate weight rooms

Losing a fight doesn’t mean you failed to follow procedure...only that you failed to complete the task.
