what about the 'lined' paper?

Common in manuscripts, but you had to rule your own lines. They also used small pinpricks to transfer lines.

Don't know about Leo, but when they cleaned up Michaelangelo's frescoes in the Sistine Chapel a few years back, they did uncover this in the Creation of Adam section:
