It's the same page, 3x, to three different people.Each one emphasizes that it is a "single, one time release". JFnK didn't put the dates of service in, just wrote "11 yrs" where the dates should be put. That seems rather odd, especially when you read the fine print on the docs, they (the military) seem rather big on dates.

I think he wrote "all years," instead of "11 years," but the "a" is small and looks like a dot.

just wrote "11 yrs" where the dates should be put. I think that chicken scratch is actually "all years"...still strange considering the question was directed towards "years(s)".
Another thing he does is mix upper and lower case letters most noticeable with the "R" in "Record" and "r" in "reporter"...etc...not sure of the significance but still a strange quirk.
