I believe it is strictly the excess glucose.

Insulin just loses its ability to shunt blood sugar into muscles and fat cells, to achieve a healthy level of glucose.

I think you're probably correct, and it makes sense as well,
the more glucose is floating around in the blood, the more its consistency is like ketchup...

...so blood can't 'get to' the smaller capillaries...
