If I were a gambling man I’d wager it’s not even moving. The training wheels keep it upright and he just sits on it long enough for a picture. There is no tension in his legs and the spokes appear to be stationary so maybe he’s riding really slow but this is clearly a photo op so he’s just sitting there pretending, in my opinion.

Ever been to Italy? In the smaller towns, it’s totally common to see octogenarian ladies zipping around on bikes (sans helmets).

I am fairly confident he’s been using toe clips for some time.
He just didn’t quite get his right foot all the way out and lost his balance. The issue is, he should still have been able to maintain his balance because he had his left foot on the ground and he was stabilized by gripping the handle bars. He’s a clutz.
The first time I used toe clips was in my 40s.
When you have to exit them the first time, the natural sideways move you grew up with, doesn't work for you.
Now, I was going a bit faster than he was, but I too fell.
