Considering the penny pinching on safety, the reports of using the gun for target shooting and the absolute disregard of common sense gun etiquette by Baldwin I’d say the odds were more like 99 to 1 of what happened happening.

The producers didn't follow safety procedures set in place and Alec didn't even bother to double check his gun. These two things both independent of one another as well as combined, is why a 'one in a trillion' incident occurred.

It was NO ACCIDENT. It was the outcome of safety procedures being dispensed of an an actor that didn't bother to double check that the weapon was safe prior to use.
