Although it is possible that person has been in temporary or term status for 4-7 years, and the 2nd level supervisor finally relented to allowing the employee to become permanent only after many requests from the 1st level supervisor.

To my knowledge, at least where I work, I can't recollect where any open positions did not have anyone pre-selected.
