It’s deliberate.
Designed as propaganda to “get” Trump.
They have a narrative, and getting Trump—and us, through Trump—is it.
100% lies all the time. Not lazy, even they are. It is much more insidious than laziness.
This time it failed, due to sharp-eyed observers but let me say that...
They do it ALL the time. We miss as much as we catch, if not more.
I cannot watch the news, read newspapers or news magazines any longer, but when I do, I always wonder what bullshit are trying to sell me?
We all need to ask the same question.

President should sign an FCC order stating that any news agency broadcasting false information is required to devote 50% of their broadcasting time apologizing for the false reporting until the next election cycle ends. Failure to comply will result in all broadcasting privileges forfeited until compliance is reached.

Broadcasting false content during news programming
The FCC is prohibited by law from engaging in censorship or infringing on First Amendment rights of the press. It is, however, illegal for broadcasters to intentionally distort the news, and the FCC may act on complaints if there is documented evidence of such behavior from persons with direct personal knowledge.

I don’t believe this is a quality control issue, or a lack of understanding and how to conduct research. It was fraud, and in their minds there’s a war on, and they just wanted dramatic photos of Tracer bullets flying. It’s like making the movie to them. We think it’s news, the truth is it’s just entertainment.
