Ruptured pulmonary artery?

Heart failure can cause a lot of coughing. My guess is all the coughing ruptured the small blood vessels in his lung.

Ruptured pulmonary artery?

I'd guess a ruptured right-sided bronchial artery. These are small arteries supply the bronchial tree and one ruptured into a right sided bronchus, filling and backfilling up the right bronchial tree with bright red blood where it then clotted.
