If you only knew the level of miscommunication and mismanagement going on...different word meanings to different education level...experience...word accents...simple human brain fogs..errors in speaking...ambient noise interference...misguided policies...human tiredness...patient indifference..patient ignorance...budget control...drug shortages...emergencies..people calling in sick..people working while ill...personnel animosities...harassment...computer failure...bad handwriting..short staffing.etc.
All above contribute to glaring errors which occur and are somehow corrected before getting to patient..but some get through...
2 times a row I tried to have a stress test. The first time they put the isotope under my skin, not in my vein, second time isotope with the wrong marker. The tech admitted she did not look at the isotope market because things had always been correct in the past.
The hospital sent me a letter stating it didn’t meet the level that had to be reported to some board.
Probably by the time I had the third test, at another hospital I had reached the level.
