Moslems in the Middle East do try to stay pure in thought and deed. Their concept of maintaining such holiness is to not lust after women. However, we men get strong urges to procreate. However, although masturbating or having sex with a woman is not holy, using a male friend's or an animal's anus as a stand in, is, because one could not possibly have amorous, sinful desires with a fellow male or an animal. In these cases, the friend or little boy are helping the dominant male stay holy, which is holy for them then, too. The animal is not made holy by having sex with it, but it does allow that “special itch” to be scratched in a needed way.
This is how I understand how conservative Moslems, especially in rural areas, properly handle their sexual urges.
That itch needs to be cut off!
