One of my sons was born on December 22. He always gets a birthday cake and birthday presents separate from Christmas.

My son born on Dec 23rd also always had a separate birthday cake, presents and party.
We just celebrated my son’s 21st on Dec. 23rd. He gets presents on his birthday and Christmas of course. I just asked him if it bothered him at all.
