If somebody’s “identity” is as something they clearly are not, are they not clearly mentally ill?

Pretty much the definition. A mental "dis"-order is when your internal reality doesn't match what actually is.

I have a mental illness. I have a form of body dysmorphic disorder where my internal "view" of my body size differs greatly from reality. I think I'm actually kind of small and average. This has led me to exercise, some times obsessively, to put on the size I feel I lack.

I still function normally in all other regards, I don't get "triggered" over stupid stuff, ect...

Others... their mental illness takes other forms. Some times, much to the detriment of others if they insist the rest of us share their delusions...
