My veins are mostly buried. Monitoring my blood sugar I get routine blood draws. I usually get blood drawn from my hand where the veins are visible rather than them poking around in both arms only to give up and take it from my hand like I told them.
Age spot. I occasionally get one that looks exactly the same.
One reasonable explanation. I can see him on blood thinners to prevent stroke or heart attack.

I’ve known EMTs and Paramedics who would give themselves Saline IVs after a tough night of drinking. A quick way to hydrate away a hangover.
That was long ago and far away.
Maybe Joe needed some juice.

He may have given labs for blood work. A few times over the years Ive had it taken from my hand when I was dehydrated and the arms were not giving enough blood.
I think you are right. And it looks too close to the knuckle for an IV wound.
Looks like an infiltration or a blown vein. Maybe there’s another site higher up. We’ll never find out.
I watched Joe’s little gathering yesterday, and if that’s Joe on uppers, he will never make it to November 3.
He reminded me of a 90-year-old giving a toast at his great grandchild’s wedding.
That is an awfully big needle puncture wound in the middle of the swelling. A normal IV drip would use a much smaller needle. That looks like a 15 gauge or more wound. The swelling really hasn’t had time to bruise to purple properly. Trust me I’m an expert on monthly infusions over the last 10 years.

I was responding to the “conspiracy theory” part of your comment, not accusing you of saying anything about liver spots, although you thinking it’s just a conspiracy by nature infers that you don’t think the spot on Joe’s hand is from an IV. Then what is it in your mind, a liver spot?

I've had several IV injections in the back of the hand. This picture is bruise evidence of such an injection site. It is NOT a liver spot.
A little body makeup and that spot disappears.

They might not have been able to find a vein. That happens to me when I become even slightly dehydrated.
Last time I was in the hospital (February) I fought like hell to not get the IV on the top of my hand because it hurts like hell to get it there, and it's painful for days afterwards.
I lost that battle and that's an IV drip mark for sure.
Now that it’s been brought to their attention they will choose a site that can be covered by the sleeve.
