Accidents had happened before in such an environment. On July 3, 1988, at the tail end of the Iran-Iraq War, an Aegis cruiser in the Persian Gulf, the USS Vincennes, fired two Standard Missiles at a commercial Iranian Airbus, IR 655.

That was not an accident.

The pilot turned off his transponder and mimicked the flight pattern of a fighter jet while making a feign attack towards a US Carrier Group.

He knew exactly what could likely happen...

"Hmmmm... let me scare the Americans with this passenger jet full of innocent people... maybe they martyr us, maybe not..."
