Please explain "Failures at nuclear missile silos". The Titan explosion in Arkansas is the only one I know of.

In nine years of Minuteman III missile duty I don't recall any others. I do remember the mystery nukes showing up at Barksdale but that had nothing to do with the ICBM fleet.

Weren’t there a few incidents in the last few years in which some missile launch officers failed some sort of readiness inspection?
