Have a plan that imposes a $10,000 annual deductible, a $50,000 maximum lifetime deductible, a $1 million annual coverage limit, and a $5 million lifetime coverage limit. You'd probably be shocked at how inexpensive this would be.

If nothing else, it would help turn health coverage in the U.S. back into real insurance -- because the insurance industry would actually be able to price their risk in this scenario.
