A lot can be explained by anorexia/malnutrition.
All the same, a lot of this would appear to be driven by the desire to make the girls look like boys (and vice versa).

Back in the '90s when Phil Gramm was running for the GOP nomination, a reporter asked him if he would consider a female running mate.
