You’re correct, by definition Boerghaave syndrome is an esophageal tear that results from vomiting. An indirect cause. He wouldn't have vomited (and torn his esophagus) if he hadn't eaten the ghost pepper.
But then, the headline "Vomiting Puts Hole in Man's Esophagus" maybe wouldn't have drawn as much attention? :-)
