Believe it or not, Dan Wesson made a revolver for a *cough* three-letter agency that included a suppressor. It was supposed to be used to assassinate Idi Amin, but the assassin was either apprehended or unsuccessful.
The barrel could be screwed down to close the gap between the cylinder and the barrel. It must have made that second shot difficult.
