I didn’t believe the denials. Using microwaves to spy on intelligence targets was an obvious possiblity to me. Side effects? These are the bad guys, why would they care?

The same way you are shielded from the light from a flashlight. It is a beam that can be pointed. Microwaves are simply a form of light itself, just like radio waves and x-rays are .
It would be both. The microwave is modulated by the sound vibrations of the surface it reflects off of. The problem is you get all kinds of reflections and therefore a mixed up return signal. To make sense out of the mixed up mess you also send an ultrasound signal. Since you know the return includes that ultrasound part it provides the information you need to unsramble the rest of the return information.
