In addition to having accumulated domestic enemies (families of their victims, Arab and Kurdish minorities, etc.), the Iranian regime also has cultivated wealthy enemies in the Gulf, who could sponsor attacks. The USA is certainly capable as well.

It’s probably from a number of foes. One emboldens the others to weaken the regime. Could be Israel, Saudi, US, UK, and local anti-government militias. A couple of blasts at their nuclear sites. 7 ships catch fire at the port. Pipeline explodes. Keeps them guessing and undermines their government in the eyes of the people. If it is just one actor, then they have done a heck of a job infiltrating.
Obama had a real chance to reach out when the Iranian people were doing mass demonstrations in Tehran - and being shot in the streets. Instead of helping the people he engineered to drop sanctions and send the government $100+ billion.

You left out one additional issue:
100,000 German Jews had fought for the Kaiser in WW1, they all knew how to fire a rifle, and before and even after they instituted the beginnings of gun control in Germany, guns were available.
