I trained my kids from day one to say the following.

“My parents have told me to not fill out ANY paperwork and to ALWAYS bring the forms home.”

They did bring home some questionable survey a couple of times, to which I wrote “no response” and signed it and returned it. Never had a problem.

In my high school - 40+ years ago - if they had given us a survey like that, the made-up answers would have given those reviewing the results immediate strokes. None of us would have taken the thing seriously, and some of the answers would have been legendary...

...and then the parents (some of whom were WW2 and Korean War vets) would have heard about it, and the police might have been very busy trying to find bodies.

I'm confused by the question. I answered a call from the CDC. It was a survey and I said heck yeah I'll answer. The 2nd question was do you describe yourself as: and she listed everything. Then she wanted to know my pronoun preferences. I have no idea what that even means and why is it relevant to Covid and the shot?
