Glimmer of hope in New York as Gov. Cuomo says rate of hospitalizations has slowed

Gov. Andrew Cuomo said early signs show social distancing is paying off, as the exponential rate of new hospitalizations has slowed.

“The evidence suggests that the density control measures may be working,” Cuomo said at a news conference on Wednesday. The number of hospitalizations, which on Sunday were doubling every two days, are now doubling nearly every five days. However, the governor showed some hesitation to believe things could slow that much.

The way I read it, the number of hospitalized cases is 3,922. I think you're reading the wrong column.

Unfortunately a few hours after this article was published some big new case numbers came out of NYC. Hopefully as someone posted we’ll see some positive results from those anti-viral drugs, unless Cuomo interferes with those drugs being prescribed by doctors.

3900 is a scary number considering that two weeks ago it was less than a 100. Yet of interest are how many are in critical condition. Neighborong Nassau county has 4000 cases and 10 critical patients. Problem in the city is that all ages are under assault. A 36 year old HS principal passed yesterday. It does not seem to be Italy.

To all medical professionals around the world:

My name is Dr. Zev Zelenko and I practice medicine in Monroe, NY. For the last 16 years, I have cared for approximately 75% of the adult population of Kiryas Joel, which is a very close knit community of approximately 35,000 people in which the infection spread rapidly and unchecked prior to the imposition of social distancing.

As of today my team has tested approximately 200 people from this community for Covid-19, and 65% of the results have been positive. If extrapolated to the entire community, that means more than 20,000 people are infected at the present time. Of this group, I estimate that there are 1500 patients who are in the high-risk category (i.e. >60, immunocompromised, comorbidities, etc).

Given the urgency of the situation, I developed the following treatment protocol in the pre-hospital setting and have seen only positive results:

1. Any patient with shortness of breath regardless of age is treated.

2. Any patient in the high-risk category even with just mild symptoms is treated.

3. Young, healthy and low risk patients even with symptoms are not treated (unless their circumstances change and they fall into category 1 or 2).

My out-patient treatment regimen is as follows:

1. Hydroxychloroquine 200mg twice a day for 5 days

2. Azithromycin 500mg once a day for 5 days

3. Zinc sulfate 220mg once a day for 5 days

The rationale for my treatment plan is as follows. I combined the data available from China and South Korea with the recent study published from France (sites available on request). We know that hydroxychloroquine helps Zinc enter the cell. We know that Zinc slows viral replication within the cell. Regarding the use of azithromycin, I postulate it prevents secondary bacterial infections. These three drugs are well known and usually well tolerated, hence the risk to the patient is low.

Since last Thursday, my team has treated approximately 350 patients in Kiryas Joel and another 150 patients in other areas of New York with the above regimen.

Of this group and the information provided to me by affiliated medical teams, we have had ZERO deaths, ZERO hospitalizations, and ZERO intubations. In addition, I have not heard of any negative side effects other than approximately 10% of patients with temporary nausea and diarrhea.

In sum, my urgent recommendation is to initiate treatment in the outpatient setting as soon as possible in accordance with the above. Based on my direct experience, it prevents acute respiratory distress syndrome (ARDS), prevents the need for hospitalization and saves lives.

I see more cases but I see fewer and fewer deaths....the ones that were in deep risk categories have sadly died....too bad someone didn't tell them a month ago to isolate...

a virus is a virus and we'll see it again and new versions...hope we're more rational next time around...

Cuomo could have had 16,000 ventilators several years ago and chose not to pay for that item.

He was the one ill prepared. It is not on Trump that Cuomo has not prepared New York for a pandemic.

The folks had on during yesterday evening’s talk made it clear that all the projections were based on us not doing anything to stop the virus for 2-3 cycles of it...actual projections are much lower and moving equipment from low-impact areas (or those who have basically finished their cycle) will give us plenty...and the deals used by anesthesiologists can be easily modified into respirators which gives us multiples of what we thought we had and we get operators with them because elective surgeries have been put on hold...

Do you think everything should have gone on as normal right from the beginning? How about even now? If you were offered an extraordinary, free vacation in New York City right now, what would your considerations be before bringing loved one’s there?

Well that is all the people in leadership, including Trump, are doing right now. My primary point is that throwing out juvenile lines like ‘Covid clowns’ might give you a little thrill up your leg when you write it, but ultimately it is childish and serves absolutely zero purpose.

Ok, you acknowledge something should have been done. So what was your plan at the very beginning? It's simple to throw out juvenile names (I think those calling people flubros are wrong also) but I'd like to see the plan you were promoting right from the start. Could you link to the thread where you told us all how to handle the situation early on?

Fwiw, I was right in your camp until recently after listening to too many experts and checking out their backgrounds and realizing we were on the same side, taking care of the United States. The giant catch 22 of your hoped for ‘day of reckoning’ is that it could only come because of the advice virtually every expert was telling President Trump and other leaders that they acted on.

Rational people don’t ignore reality and facts. Although I don’t use the term “FluBros” that is what many of them have been doing. Posting irrelevant stats and information about past flu seasons does not an argument make. It is completely irrelevant. People who do that should not be expected to be taken seriously. What has been happening and is now happening in other parts of the world has been very real that certainly doesn’t occur with the seasonal flu - that’s why policy steps are being taken to avoid having that happening here - if Italy had restricted travel from China perhaps they wouldn’t be having the situation they are having now (and obviously if China had been upfront about this and voluntary restricted travel until it got it under control none of this would be happening most likely). Entire large clusters of infections here in the US would never have materialized but for a few individuals simply engaging in unwise behavior. The President and other leaders took measures earlier here than other places did to combat this, watching what was happening elsewhere, in order to try to get “ahead of the curve.” The steps taken by the President and other leaders have been extraordinary to combat this. Let’s pray that it works - and that more treatments come on line to further push down the curve and allow for quicker recovery - slowing the rate of infection buys time for this. Of course if it does have a significant positive effect, there will be people claiming the policies had no impact and that the infections would never have occurred...but that would be fine with me...it would mean this is over and we beat this enemy.

Yes, it’s a strawman because like other rational people here, I’ve never called for doing “nothing.” What’s usually done during flu season? Advise caution, promote hand washing and discourage going out when sick or just recovered, protect the weak and elderly. Add in voluntary work from home, shut down travel to and from China, close airports where outbreaks occur, close senior centers and rest homes to visitors, etc. Not shut the whole economy down cold!

Yeah, those experts were giving great advice!

I think we will be better prepared in future. The disruptions & shortages are in large part because:
A. BO didn’t restock the storehouses after the wide scale flu pandemics on his watch
B. The CDC response team was not adequate for an emergency and it’s mandate had shifted to politically correct issues like obesity
C. The lessons of the past pandemics were lost on Presidents who didn’t care about Americans and went back to business as usual
D. These same “leaders” allowed and encouraged America to become a service economy & off-shored the essential supply chains for drugs & supplies
The inability of the CDC & response team to be quickly out of the box in the face of an unknown virus which had shown significant morbidity, mortality and an enhanced ability to spread mandated a Nationwide response to allow time for the tests, supplies and manpower to be deployed.
The important thing is always the downside risk. At the onset that risk was very high and warrented assuming some economic risk. Now that the response is in place, there appear to be some effective drug therapies and the virus is behaving as a less lethal pathogen the measures deployed to slow the spread will be rolled back.
The next time we will not be starting from behind the 8-ball & perhaps with POTUS shaking the tree we can cut the numbers of seasonal flu deaths by a large %. The flu shots will never be the answer. There are too many strains and the vaccine makers guess 47% right in their best year. There are studies showing that people who get the flu shot are more susceptible to strains not in the shot. If we put the effort into testing drug therapies we may be able to find not only effective therapies for the sick but safe drugs that can protect vulnerable populations from influenza & coronaviruses.

That's exactly right. But the President didn't need "experts" to simply view what is going on in Europe and their failure to act early and restrict basic things like travel to know what can occur without decisive action. He can see it with his own eyes what could have occurred here, as can anyone else.
