The Captain has an emergency access code to gain entry when the cockpit door is locked. It can be intentionally disabled from within the cockpit.

The fact that he actively over rode the Captain’s emergency access to the cockpit. The fact that he purposely took the plane out of autopilot and purposely flew it directly into a mountain.

I was speculating along these lines as well. Did Lubitz fly with this particular crew before?
