Not sure if it is just plain racist (black person/banana) or they are also taking a shot at his homosexuality that the press has kept a tight lid on here in the US.

Since he's depicted as fellating the banana, it's got to be aimed at his sexual orientation as well. Indeed, it corroborates all the rumors about his sexual orientation. Since he's half black, it's the absolutely vilest public insult imaginable, an insult saying they have so much really filthy dirt on him that they know he can't retaliate without destroying himself.
