The top 10% and many in the lower economic levels who have a little foresight. I advise all my coworkers, most of whom are under 40 to investigate international insurance and to bone up on medical tourism. Several have been doing so. Interestingly, the ones who have got into it the most are black. One young woman and her husband have even got passports and have located a distant relative with land in the Bahamas. They are investigating offshoring themselves and he has already lined up work in Nassau. These folks are security guards with two small children, as I am. They are not in the top 75%.
