Manhole covers dont generally explode.
I would speculate that something under the manhole cover exploded.
Under the manhole? I believe that's a common place to visit in SF.

Well, technically, yes.
It's been a routine occurance in Washington DC the last 5 years ago; I think it's happened about a dozen times. The utility infrastructure is very old and deteriorated in NW DC, and for various reasons including gas leaks, electrical fires, etc. Manhole covers have literally exploded out of the street.
