San Francisco fire captain Pete Howes told KCBS a man hole cover in front of the Ralph Lauren store at the corner of Post and Kearny appeared to have exploded,

Manhole covers don’t generally explode.

I would speculate that something under the manhole cover exploded.

We had a massive gas leak here in Alaska a couple of days ago, but she left.

Well, technically, yes.

It's been a routine occurance in Washington DC the last 5 years ago; I think it's happened about a dozen times. The utility infrastructure is very old and deteriorated in NW DC, and for various reasons including gas leaks, electrical fires, etc. Manhole covers have literally exploded out of the street.
