Swift boats weren't big enough to have doctors on the boat, so of course he wasn't a crewmate. If this was for the first purple heart, the "doctor" who signed off was a hospitalman first class, i.e. a medic. Sorry you don't get a doctor to sign off for a metal fragment the size of a matchstick.

I never had the impression that this veteran was a doctor. I understood he provided medical treatment, like a physician's assistant or a paramedic in the non-military arena. I'm not sure of the correct military term, but I never thought-- and I don't think he ever stated-- that he was a doctor.

On Kerry's own website, he has a PDF file with two of the messages filed from the Medical Officer who treated him. In both cases he was treated by the Medical Officer on a Coast Guard cutter. Both times he was IMMEDIATELY returned to duty. Once his prognosis was listed as good, the other excellent. Those were for the 2nd and 3rd Purple Hearts. They also said do not notify next of kin. All of which indicates the wounds were very minor.
The first wound only needed Bactine according to the doctor who treated Kerry.
