I think the most reasonable explanation, given that Intelligence is matching up this voice with bin Laden CONSISTENTLY, is that bin Laden has changed his appearance. Any videotaping would render that obsolete. There would be absolutely no good reason for him to show up on television. AQ is obviously getting along fine....

Don't be so sure.

He knows we have used video tapes in the past to pin down the location they were made. In addition, we know some of his video tapes showed such bad health that it was considered demoralizing to his supporters.

Or, perhaps he shaved his beard as part of a survival motivated disguise. Any change in his appearance would need to be kept a very tight secret. (A fattened up, clean shaven Osama in a business suit? He'd walk right past airport security.)

Finally, (pause... putting on tinfoil hat...): What if the infamous "go" code is something as simple his doing exactly what you just described? (Whatever the "go" code is, you can bet it will be simple.)
