Who did the thwarting of the mission and why? I know it's fiction, but Ollie is probably couching the truth in the book.

No, North says the book depicts actual events and that only the names have been changed. The mission was compromised by the joint efforts of the UN and the shared encription technology used by the military. The technology treason involved all the usual suspects for all the usual reasons.
